If your most recent subscription was maintained for less than 4 months and you cancel it, you will have to pay the $75 initialization fee again if you decide to start a new subscription. 

If your last subscription was 4 months or longer and terminated by you (not because of a failed payment), please email [email protected]  to request instructions for how to have your initialization fee waived.
